The Colville Tribal Diabetes Program's mission is to provide the highest quality, culturally competent health education services to each community in a collaborative manner which promotes a safe and healthy environment. Our program focuses on providing preventative education tools for people through advocacy of healthy lifestyles that honor mind, body and spirit.
SERVICES
For any services you will need current diabetes prescription, be an enrolled member of a federally recognized tribe, and be eligible for Indian Health Services.
Shoes: No current shoe store. Contact Alana or Charlene for Berg’s referral; assisting clients with $150.00 credit toward shoes.
Glasses: Through Indian Health Services, we are assisting clients with $150.00.
Dental: Through Indian Health Services, we are assisting clients with $250.00
Reminder: Contact Alana Epperson at our office for pre-approval of services. You can receive these services 1 time per year.

Instructions: This form will be completed by Diabetes Prevention staff in coordination with medical personnel to enroll you in services.

methods & goals
Increasing access to quality diabetes education and management services across the Colville Reservation through:
Developing a standard approach to care and management of diabetes, including screening, based on the current SDPI (Special Diabetes Program for Indians) best practice guidelines, and an adapted to use in the Tribal communities
Establishing effective community-directed approaches that revolve around the prevention and managing of diabetes-related complications
Developing and promoting cost effective ways of delivering quality diabetes education through life cycle of the grant
